At the type locality, the Uchucchacua Mine, Oyon Province, Lima, Peru, uchucchacuaite occurs in a telescoped hydrothermal deposit associated with alabandite, galena, benavidesite, sphalerite, pyrite, pyrrhotite and arsenopyrite (HOM).
