Koutekite occurs in arsenical copper deposits associated with paxite, arsenic, native silver, skutterudite, nickeline, löllingite, chalcocite, algodonite, domeykite, allargentum, kutinaite and calcite (HOM). It is replaced by löllingite (AM46.467).

At Černý Důl, Krkonoše Mountain, Czech Republic, koutekite occurs with arsenic, native silver, skutterudite, löllingite and chalcocite (AM43.794).

In Kazakhstan, koutekite occurs with domeykite, löllingite, native silver and nickeline in copper-mineralised sandstone. Intergrowths of koutekite with domeykite and löllingite occur. Koutekite often replaces domeykite (AM78.677).
